Preparation for your first appointment with us
We want to prepare ourselves optimally for you and your partner. It is therefore very important for us to receive a medical history form from you in advance. Please take your time and complete this form carefully.
You can fill in the form online and send it directly to us (it will be transmitted in encrypted form) or you can print it out (as a PDF) and fill it in conventionally. In any case, we need the medical history forms of both spouses/partners a few days before the planned appointment at our practice.
